  {"id":18464,"date":"2022-03-22T14:37:00","date_gmt":"2022-03-22T06:37:00","guid":{"rendered":"https:\/\/www.curtin.edu.au\/friends\/?p=18464"},"modified":"2025-07-24T12:38:55","modified_gmt":"2025-07-24T04:38:55","slug":"pilot-study-provides-glimmer-of-hope-to-those-with-parkinsons","status":"publish","type":"post","link":"https:\/\/www.curtin.edu.au\/friends\/pilot-study-provides-glimmer-of-hope-to-those-with-parkinsons\/","title":{"rendered":"Pilot study provides glimmer of hope to those with Parkinson\u2019s"},"content":{"rendered":"\n\n\n<p class=\"has-sm-font-size\"><em>One of the first symptoms of Parkinson\u2019s is a tremor in your hand.<\/em><\/p>\n\n\n\n<p class=\"has-sm-font-size\"><strong>Author | Carmelle Wilkinson<\/strong><\/p>\n\n\n\n<p>Have you ever experienced a slight tremor in your finger, thumb, hand or chin?<\/p>\n\n\n\n<p>Shaking can be normal side effect of exercise, low sugar, injury, stress or medication \u2013 but in some cases it can also indicate an early sign of Parkinson\u2019s.<\/p>\n\n\n\n<p>Twenty years ago, Donald Main noticed a slight \u201cjerk\u201d of his hand which prompted him to seek medical advice.<\/p>\n\n\n\n<p>While very small and barely noticeable to others, the little tremor was a nagging reminder that something wasn\u2019t right.<\/p>\n\n\n\n<p>Following medical investigation, Donald was diagnosed with Parkinson\u2019s at 67 years of age.<\/p>\n\n\n\n<p>Newly retired, labourer Donald had been looking forward to years of pottering about his garden in Wilson.<\/p>\n\n\n\n<p>\u201cThe diagnosis came as a shock. I wasn\u2019t expecting that. I guess you could say it slowed me down,\u2019\u2019 he said.<\/p>\n\n\n\n<p>\u201cI was a sheet metal worker for many years working in a factory, so I used my hands a lot.<\/p>\n\n\n\n<p>\u201cIt\u2019s been a slow progress since I was first diagnosed, but over the years I have noticed difficulty with walking, and I also struggle with speech.\u201d<\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-style-default\"><img decoding=\"async\" src=\"https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/donald.jpg\" alt=\"\" class=\"wp-image-9217\" \/><\/figure>\n\n\n\n<p class=\"has-sm-font-size\"><em>Donald Main in his garden at home.<\/em><\/p>\n\n\n\n<p>It\u2019s this difficulty in speaking that prompts his daughter and full-time carer Darlene Main to take over the interview for him.<\/p>\n\n\n\n<p>\u201cDad finds it hard speaking for too long now. But the hardest thing to see is when he is experiencing what we call a freeze. Where he\u2019ll be slowly walking in one direction, and if he wants to change direction his body just stops and thinks about it for a few seconds before moving,\u2019\u2019 she said.<\/p>\n\n\n\n<p>\u201cEven though Parkinson\u2019s has slowed Dad down, it hasn\u2019t dampened his spirits. Dad is still his old chirpy self and likes to get out in the garden any chance he gets.<\/p>\n\n\n\n<p>\u201cWhile he doesn\u2019t move around as much as he used to do, he does enjoy taking slow walks down the street using his walking frame and he enjoys helping me cook.\u201d<\/p>\n\n\n\n<p>A recent fall at his property in Wilson saw Donald receive medical attention for grazes to his arm.<\/p>\n\n\n\n<p>\u201cThat\u2019s what worries me the most. Dad\u2019s balance and coordination isn\u2019t as good as it used to be,\u2019\u2019 she said.<\/p>\n\n\n\n<p>Donald is one of 80,000 people in Australia living with Parkinson&#8217;s disease, with one in five being diagnosed before the age of 50.<\/p>\n\n\n\n<p>It\u2019s the second most common neurodegenerative condition in Australia after dementia and causes shaking in the hands, arms, legs and jaw, stiffening limbs, slowed movement and impaired balance and coordination.<\/p>\n\n\n\n<p>As the disease progresses, people may experience difficulty walking and talking, mental and behavioural changes, sleep problems, depression, memory difficulties, and fatigue.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-style-default\"><img loading=\"lazy\" decoding=\"async\" width=\"2560\" height=\"2560\" src=\"https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/shutterstock_1707396892.jpg\" alt=\"\" class=\"wp-image-9218\" srcset=\"https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/shutterstock_1707396892.jpg 2560w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/shutterstock_1707396892-150x150.jpg 150w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/shutterstock_1707396892-768x768.jpg 768w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/shutterstock_1707396892-1536x1536.jpg 1536w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/shutterstock_1707396892-2048x2048.jpg 2048w\" sizes=\"auto, (max-width: 2560px) 100vw, 2560px\" \/><\/figure>\n\n\n\n<p>Another common and vexing symptom in people with Parkinson\u2019s and other neurological disorders such as multiple sclerosis, cerebral palsy and stroke is drooling.<\/p>\n\n\n\n<p>Affecting up to 74% of people who have Parkinson\u2019s it has been ranked as one of the most bothersome symptoms, causing social withdrawal.<\/p>\n\n\n\n<p>Donald recently participated in a pilot study run by Curtin Senior Lecturer and Parkinson\u2019s researcher <a href=\"https:\/\/staffportal.curtin.edu.au\/staff\/profile\/view\/naomi-cocks-bf0f97d0\/\" target=\"_blank\" rel=\"noreferrer noopener\">Dr Naomi Cocks<\/a> to control excessive drooling.<\/p>\n\n\n\n<p>Funded by <a href=\"https:\/\/www.parkinsonswa.org.au\/\" target=\"_blank\" rel=\"noreferrer noopener\">Parkinson\u2019s WA<\/a> and trialled in conjunction with <a href=\"https:\/\/parkc.co\/\">ParkC<\/a> and Royal Perth Bentley Group, behavioural treatment known as Expiratory Muscle Strength Training (EMST) was used to strength the muscles used for swallowing and coughing.<\/p>\n\n\n\n<p>\u201cImagine not wanting to leave your house because of fear you would drool. That\u2019s the choice many people with Parkinson\u2019s disease confront every day. Drooling can lead to a number of problems including social isolation, \u2018\u2019 she said.<\/p>\n\n\n\n<p>\u201cThe misconception is that excessive saliva causes drooling, when in actual fact those with Parkinson\u2019s don\u2019t produce more saliva, they just don\u2019t have a great swallow, and this is due to weakened muscles and decreased lip closure.<\/p>\n\n\n\n<p>\u201cWhile treatment to stop production of saliva, such as botox injections, have been effective for some people, in others it actually causes more problems such as dry mouth, making it even harder to talk or swallow.\u201d<\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-style-default\"><img loading=\"lazy\" decoding=\"async\" width=\"2560\" height=\"1707\" src=\"https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/IMG_2601-min.jpg\" alt=\"\" class=\"wp-image-9219\" srcset=\"https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/IMG_2601-min.jpg 2560w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/IMG_2601-min-768x512.jpg 768w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/IMG_2601-min-1536x1024.jpg 1536w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/IMG_2601-min-2048x1365.jpg 2048w\" sizes=\"auto, (max-width: 2560px) 100vw, 2560px\" \/><\/figure>\n\n\n\n<p class=\"has-sm-font-size\"><em>Curtin Senior Lecturer and Parkinson\u2019s researcher, Dr Naomi Cocks<\/em>.<\/p>\n\n\n\n<p>Saliva is essential to keeping our mouth clean.&nbsp; Naomi said dry mouth can also lead to the build-up of bacteria, which could lead to pneumonia if the person has an impaired swallow and the bacteria goes into the lungs.<\/p>\n\n\n\n<p>Following consultation with people with Parkinson\u2019s, and health professionals who work with in the field, Naomi discovered the need for greater research on alternative treatment options for drooling.<\/p>\n\n\n\n<p>\u201cThere is a growing body of research which has found that expiratory muscle strength training improves cough and swallow in people with Parkinson\u2019s so we were excited to trial this,\u2019\u2019 she said.<\/p>\n\n\n\n<p>Naomi\u2019s recent study involved 16 participants with Parkinson\u2019s undertaking expiratory muscle strength training exercises multiple times a day.<\/p>\n\n\n\n<p>Training consisted of two sessions a week with a speech pathologist and three sessions on their own at home.<\/p>\n\n\n\n<p>First developed for high school band students to increase exhalation strength EMST is a bit like blowing into a large straw but instead of the breath passing freely through the straw, the air encounters resistance which helps strengthen the target muscles.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large is-style-default\"><img loading=\"lazy\" decoding=\"async\" width=\"1000\" height=\"500\" src=\"https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/EMST-1000x500.jpg\" alt=\"\" class=\"wp-image-9220\" srcset=\"https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/EMST-1000x500.jpg 1000w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/EMST-740x370.jpg 740w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/EMST-480x240.jpg 480w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/EMST-1260x630.jpg 1260w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/EMST-1520x760.jpg 1520w, https:\/\/www.curtin.edu.au\/friends\/wp-content\/uploads\/sites\/2\/2022\/03\/EMST-1920x960.jpg 1920w\" sizes=\"auto, (max-width: 1000px) 100vw, 1000px\" \/><\/figure>\n\n\n\n<p class=\"has-sm-font-size\"><em>Expiratory Muscle Strength Training (EMST)<\/em> <em>apparatus is used to strengthen muscles for swallowing and coughing.<\/em><\/p>\n\n\n\n<p>\u201cMuch like a gym workout they were asked to repeat this exercise five times a week, several times a day for a period of six to eight weeks,\u2019\u2019 Naomi said.<\/p>\n\n\n\n<p>\u201cAfter training, we measured their swallowing ability, cough strength and drooling and we found they had all improved significantly. \u201cThis is an important treatment for people with Parkinson\u2019s, as a weak cough and impaired swallowing places them at increased risk of aspiration pneumonia and choking.\u201d<\/p>\n\n\n\n<p>Following high patient satisfaction and no adverse side effects, Naomi is now looking to widen her research to encompass a larger study group.<\/p>\n\n\n\n<p>\u201cWe are now seeking funding in order to explore this treatment\u2019s efficacy with a greater number of participants in a randomised control trial,\u2019\u2019 she said.<\/p>\n\n\n\n<p>\u201cWe hope our research will help people with Parkinson\u2019s feel more comfortable with leaving their homes and, in doing so, improve their quality of life. Our aim is to recruit 70 people with Parkinson\u2019s over a three-year period.\u201d<\/p>\n\n\n\n<p>Donald said the trial had made a huge impact on his quality of life and he continued to use EMST several times a week.<\/p>\n\n\n\n<p>\u201cI don\u2019t get out much these days, but it definitely helped with the drooling. I don\u2019t even notice it much anymore to be honest.
