Please let us know in advance if you have any access requirements for your appointment (e.g. Auslan interpreter).<\/p>\n\n\n\n<p>We have services available for on-campus, regional, and online students.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\"><strong>Service hours<\/strong><\/h2>\n\n\n\n<p>Monday to Thursday: 8.30am to 4.30pm<br>Friday: 8.00am to 4.00pm<br>Please note:&nbsp;There may be a waiting period during our busy times of the year.<\/p>\n\n\n\n<p>If you are looking to obtain a Curtin Access Plan, we encourage you to make the appointment <strong>prior to the start of the study period or as early in the study period as possible<\/strong> as they may be long appointment wait times when the study period is underway.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Who can receive assistance?<\/h2>\n\n\n\n<p>You&#8217;re eligible for assistance if you have a disability, physical or mental health condition that is diagnosed by a doctor or other qualified health service provider, and it affects your studies. You can also get assistance if you have a significant caring role for a person with a disability (relative or friend) or are pregnant with medical complications.<\/p>\n\n\n\n<p>See the <a href=\"https:\/\/www.curtin.edu.au\/students\/personal-support\/disability\/definition\/\">What is a disability?<\/a> page for the definition of a disability as well as common disabilities encountered at university.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"documentation\">Health Professional documentation<\/h2>\n\n\n\n<p>If you require disability support and&nbsp;&#8216;reasonable adjustments&#8217; you must provide a letter or statement from an appropriately qualified health professional or a completed <a href=\"https:\/\/www.curtin.edu.au\/wp-content\/uploads\/sites\/8\/2021\/10\/Health-Professional-Report-NEW-18.12.2019.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Health Professional Report form [.pdf 220kb]<\/a> which provides information about your disability or medical condition and how it impacts on your studies. All documentation must be provided in English.<\/p>\n\n\n\n<p>Please scan your documentation and email it to <a href=\"mailto:access.ability@curtin.edu.au\">access.ability@curtin.edu.au<\/a> prior to your appointment.<\/p>\n\n\n<mimas-block \n        block-name=\"accordion\"\n        role=\"complementary\"\n        aria-label=\"Accordion content\"\n        data-segment=\"all\">\n\n                \n\n                    <div class=\"accordion\" aria-labelledby=\"accordion-title-List-of-appropriately-qualified-health-professionals\">\n            <button type=\"button\" class=\"accordion__heading\" aria-controls=\"List-of-appropriately-qualified-health-professionals\" aria-expanded=\"false\">\n            <div class=\"title\" id=\"accordion-title-List-of-appropriately-qualified-health-professionals\">List of appropriately qualified health professionals<\/div>\n            <div class=\"icon\"><\/div>\n        <\/button>\n        <div class=\"accordion__content\" id=\"List-of-appropriately-qualified-health-professionals\" aria-hidden=\"true\">\n                    <div class=\"accordion__item__content\">\n<p>Appropriately qualified health professionals include:<\/p>\n<ul>\n<li>General practitioners<\/li>\n<li>Medical specialists<\/li>\n<li>Psychologists<\/li>\n<li>Occupational therapists<\/li>\n<li>Physiotherapists<\/li>\n<li>Speech pathologists<\/li>\n<li>Social workers<\/li>\n<li>Optometrists<\/li>\n<\/ul>\n<p>Acceptable documentation for specific learning disabilities should be provided by:<\/p>\n<ul>\n<li>A psychologist (preferably with educational and\/or developmental training) in the identification of dyslexia, language-based dysgraphia or dyscalculia.<\/li>\n<li>An occupational therapist in the diagnosis of motor-based dysgraphia or developmental coordination disorder.<\/li>\n<li>A speech pathologist for difficulties related to specific language impairments or childhood apraxia of speech.<\/li>\n<li>For Scotopic sensitivity syndrome and Irlen syndrome, we will accept the Â鶹ֱ²¥ Health Professional Report of documentation signed and dated on the letterhead of the registered health professional for reasonable adjustments such as coloured paper and appropriate lighting for exams.\n<p>If further adjustments are requested beyond these adjustments, AccessAbility Services will request evidence of an adult psychometric learning assessment conducted by a relevant professional, such as a psychologist, using reliable, valid and standardised tests.<\/li>\n<\/ul>\n<\/div>\n\n        \n                            \n                    <\/div>\n<\/div>\n                    <div class=\"accordion\" aria-labelledby=\"accordion-title-How-current-does-your-documentation-need-to-be?\">\n            <button type=\"button\" class=\"accordion__heading\" aria-controls=\"How-current-does-your-documentation-need-to-be?\" aria-expanded=\"false\">\n            <div class=\"title\" id=\"accordion-title-How-current-does-your-documentation-need-to-be?\">How current does your documentation need to be?<\/div>\n            <div class=\"icon\"><\/div>\n        <\/button>\n        <div class=\"accordion__content\" id=\"How-current-does-your-documentation-need-to-be?\" aria-hidden=\"true\">\n                    <div class=\"accordion__item__content\">\n<p>Health professional\u00a0documentation must meet the following currency requirements:<\/p>\n<ul>\n<li>For temporary or changing conditions, you\u00a0may be required to submit new documentation each semester or when your Curtin Access Plan expires.<\/li>\n<li>For permanent conditions, you must submit documentation initially, and thereafter only if your condition or support needs change.<\/li>\n<li>For learning disabilities, your documentation must be dated within the past three years if the assessment was conducted before you turned 16.
