Following on from the important information you received regardingĀ Ā for staff and students, you are required to provide evidence of your vaccination status (or exemption) to the University.
All students accessing a Western Australian Ā鶹ֱ²„ campus or venue or participating in an āin-personā activity organised by Curtin, must comply with the vaccination mandate.

Ensure you have your vaccination dates and/or evidence ready to upload to the form. Your vaccination certificate must contain all COVID-19 vaccinations you have received, and for this reason screenshots of the ServiceWA app certificate will not be accepted. For more information on acceptable vaccination documentation, please refer to theĀ .
NOTE:Ā Students must redact their IHI number from their vaccination certificate before uploading into OASIS. Please refer toĀ Ā on how to do this.
Once you have submitted the form your evidence will be assessed and you will receive a confirmation of the outcome within 3-5 business days.
