What is RISE?

What can I report in RISE?

Please report in RISE (within 24 hours or as soon as possible):

Hazards聽(anything that has the potential to cause ill health or injury)
Injuries/illnesses聽(events that cause an illness or injury requiring medical attention).
Near-misses聽(an occurrence that might have led to an injury or illness, danger to someone鈥檚 health, and/or damage to property or the environment.).
Property damage
Psychosocial incidents and hazards聽(reports will remain confidential)
- A psychosocial hazard is a hazard that 鈥�
  (a) arises from, or relates to 鈥�
  (i) the design or management of work; or
  (ii) a work environment; or
  (iii) plant at a workplace; or
  (iv) workplace interactions or behaviours;
  and
  (b) may cause psychological harm (whether or not it may also cause physical harm).

What doesn鈥檛 need to be reported in RISE?

What happens to my incidents and hazards reported in CHARM? (submitted prior to 1 July 2025)

From 1 July 2025 new hazards and incidents will be reported and actioned in the new RISE system.

Incidents reported prior to this date will remain in CHARM and will continue to be accessible until all historical reports have been finalised and the actions closed out. No historical data will be lost or become unavailable.

What training is available?

To assist you to navigate RISE, Health and Safety are running training sessions (face to face and online).聽Staff can register for RISE training via with the option of online or face to face sessions available (45 mins).

How do I report an incident or hazard in RISE?

Follow the steps below to report incidents or hazards in RISE:

with your Curtin credentials.
Once in RISE, click on the Main Menu icon located at the top left-hand side of the page.
Select ‘Report Something’ and choose the ‘Incident and Hazard report’.
Complete the details of the report form by completing all mandatory fields.
Verify that the Manager/Supervisor is correctly assigned.
Add attachments (if needed).
Submit the incident or hazard report.

What happens if my line manager is incorrect?

Who can edit the incident report after submission?

You can still make changes to your incident or hazard report after submission if the status is ‘New’ or ‘Open’.

You can access your submitted report by:

with your Curtin credentials.
Clicking on the Main Menu icon located at the top left-hand side of the page.
Click the 鈥楳y Data鈥� tile.
Locate your submitted report.

The assigned person responsible for the management action plan can also edit the report while it remains in those statuses.

How do I complete the Management Plan in RISE?

The Assigned Manager is responsible for completing the management plan by completing the investigation and action plan for the assigned report.

When you have been assigned an incident or hazard report to manage, you will receive an email. The ‘View Report’ link in the email will open the relevant incident or hazard report for you.

You can also access the report by:

with your Curtin credentials.
Clicking on the Main Menu icon located at the top left-hand side of the page
Click the 鈥楳anage鈥� tile
Locate the report assigned to you and then click the … button then the 鈥楳anage鈥� button

Where can I access my draft report?

A report will be in draft if it has been saved as a draft but not submitted. Your draft reports will be displayed in the ‘My Data’ Register. Your draft report is only visible to yourself until you submit it.

You can also access your draft report by:

with your Curtin credentials.
Clicking on the Main Menu icon located at the top left-hand side of the page
Click the 鈥楳y Data鈥� tile
Under the ‘status’ section in your Register, you can view the status of each report: draft, new, open, closed, cancelled.
Click on the draft report you wish to continue working on.

How do I close an incident report?

How do I reopen an incident report?

An incident or hazard report with the status ‘closed’ can be reopened by the Manager/Supervisor assigned to manage the report or the Health and Safety Team.

To reopen your report as the Assigned Manager:

with your Curtin credentials.
Clicking on the Main Menu icon located at the top left-hand side of the page
Click the 鈥楳anage鈥� tile

Your assigned incident and hazard reports will be displayed in the Manage Register.

Find the relevant report, click on ‘…’ and select ‘Reopen’.

How do I transfer a report to someone else to manage?

You may transfer a management action plan to another person if you have received this incident or hazard report in error, or if you need to transfer responsibility to another person.

When you have transferred a management action plan to another person, they will see it in their ‘Manage’ register.

Please note that once the management action plan has been transferred, you will no longer be able to see or manage it.

To transfer a report to another person to manage:

with your Curtin credentials.
Clicking on the Main Menu icon located at the top left-hand side of the page
Click the 鈥楳anage鈥� tile
In the Manage register, locate your report and click on the 鈥�…鈥� button and then ‘Transfer’.
Search for the person you wish to transfer the report to and include transfer notes.
The transferred person will receive an email notification notifying them that they have been transferred the report.

How do I invite someone to a Management Plan?

You can invite other people to assist you to update a management plan. This may be a subject matter expert or your local Health and Safety Representative. The invite function allows you to invite people to give them edit access to the management plan while you keep management responsibility.

To invite a person to assist you to complete the management plan:

with your Curtin credentials
Clicking on the Main Menu icon located at the top left-hand side of the page
Click the 鈥楳anage鈥� tile
In the Manage register, locate your report and click on the 鈥�…鈥� button and then ‘Invite’ to add an invitee. Invitees can edit the management action plan but will not be able to make changes to the incident or hazard report.

How do I add notes and attachments to a Management Plan?

The ‘Notes’ icon is displayed at the top of the management plan.

The Notes allow the person responsible for the management action plan as well as anyone invited to the management action plan and the Safety Team members to add, view and edit brief notes for easy reference. The Notes are not accessible by the person reporting the incident or hazard. Please remember that Notes cannot be marked confidential.

How do I complete a task assigned to me from a management plan?

When you are assigned a Task, you will get an email. A notification will also appear on the ‘My Tasks’ icon in the global navigation bar.聽To complete the task.

Tasks will be listed in the 鈥�To Do鈥�, 鈥�顿辞颈苍驳鈥� or 鈥�顿辞苍别鈥� columns based on their status.

To complete a task, click the relevant task card and drag it from the 鈥楾o Do鈥� or 鈥樁俅蔷辈圆碘€� column into the 鈥樁俅遣员疴€� column, and mark Yes to the pop-up message.
